Job Description :
We are seeking a compassionate and dedicated Director of Social Services to implement and oversee social services programs under the direction of the Administrator. You will be responsible for assessing residents' social service needs, coordinating with the interdisciplinary team (IDT), and ensuring that residents receive necessary behavioral health care and services. This role also involves working with families, assisting with discharge planning, and connecting residents to community resources. Additionally, you will participate in resident council meetings, maintain compliance with federal and state regulations, and collaborate with staff to ensure the highest standard of care.
Responsibilities :
- Implement and oversee social services programs under the direction of the Administrator.
- Conduct assessments of individual social services needs and resources.
- Assist in obtaining resources from community agencies to meet residents’ needs.
- Coordinate with the interdisciplinary team (IDT) on social services activities.
- Help residents and families with financial assistance, discharge planning, and community referrals.
- Ensure residents receive behavioral health care and psychosocial support in accordance with their care plan.
- Participate in resident / group council meetings as requested.
- Provide residents with information on their rights, living wills, etc.
- Stay current on federal and state regulations, and recommend changes to policies as necessary.
- Assist residents with limited English proficiency (LEP), including translation services.
- Maintain reference materials and adhere to professional standards and regulations.
- Participate in facility surveys as necessary.
