Job Title : MDS Coordinator
Facility : Bedrock Rehabilitation and Nursing Center at Winter Park
Location : Maitland, Florida
Shift : Full Time
Pay Rate : $39–$41 per hour
Key Responsibilities :
- Ensure Quality Care : Oversee care standards to ensure our long-term residents receive the highest level of support.
- Conduct Assessments : Perform detailed assessments and interviews for new residents to develop customized care plans.
- Lead Care Planning : Design individualized care plans with measurable goals for both short-term and long-term resident improvement.
- Collaborate Across Teams : Facilitate interdisciplinary meetings with healthcare professionals, families, and caregivers to create well-rounded care approaches.
- Community Engagement : Work closely with specialists to drive the best possible outcomes for our residents.
- Provide Leadership & Education : Lead nursing management meetings and deliver in-service training on MDS, RAPS, and Triggers, ensuring staff stay informed and proactive.
- Maintain Compliance : Keep the MDS / Care Planning schedule on track, focusing on non-Medicare residents.
