Nurse Case Manager (RN)

Responsibilities

· Create individualized care plan and coordination.

· Perform ongoing evaluations to ensure patient progress toward desired outcomes.

· Work collaboratively with physicians, providers and multidisciplinary treatment team.

· Addresses and resolve client barriers using advanced conflict resolution skills.

· Monitor client progress and develop plan of client care to meet the ongoing needs of the client.

· Communicate to third party payers maintaining continuity of care for client.

· Provide and coordinate resources and available to patients and family members.

Qualifications

· RN License Required, current unrestricted Michigan

· Graduate of an accredited nursing or health care program

· 2-3 years of experience, preferred in ER, ICU, Rehabilitation or Case Management

· Certified Case Management (CCM) preferred

Skills

· Excellent critical thinking, communication and interpersonal skills

· Ability to work independently

· Strong organizational and time management skills

· Proficient in Microsoft Office
