We are currently seeking a dedicated and organized Medication Specialist to join our team. This role is essential in supporting tenants with their medication management and ensuring that all medication practices are followed accurately and safely.
Hours of Work:
- Full-time, 40 hours per week
- Non-Exempt
Minimum Qualifications:
- High School Diploma or equivalent
- At least one year of experience in a health-related field and human services
- Proficiency with MS Word and Excel

Program Manager
Key Responsibilities:
- Case Load Management: Oversee a designated caseload of approximately 25 tenants, ensuring that their medication needs are met.
- Medication Supervision: Supervise and monitor all medication management procedures, ensuring accuracy and adherence to protocols.
- Medication Ordering & Refills: Order medications and handle medication refills in a timely manner.
- Medication Counts: Conduct weekly medication counts to ensure all medications are accounted for.
- Consultations: Work closely with physicians, treatment providers, and pharmacies regarding medication concerns.
- Prescription Management: Obtain prescription orders and review provider appointment forms and hospital discharge documentation for medication changes.
- Medication Documentation: Log in medications upon delivery and update medication sheets to reflect any changes throughout the month.
- Tenant Education: Educate tenants about their medications, including names, dosages, and potential side effects.
- Documentation & Reporting: Document medication supervision outcomes and any significant events. Provide reports to supervisors about issues or behavioral concerns.
- Crisis Intervention & Stabilization: Provide coordination and assistance in crisis intervention and stabilization as necessary.
- Appointment Coordination: Schedule appointments and accompany tenants to appointments or activities as needed.
- Staff Communication: Ensure that all medication changes are communicated effectively to staff (both written and verbal).
- Collaboration: Work collaboratively with internal teams, external entities, family members, and medical/behavioral health providers to ensure comprehensive care for tenants.
- Case Record Documentation: Ensure accurate and timely completion of case records in the Agency’s electronic health record system.
- Staff Meetings: Attend regular staff meetings to discuss program updates and address any relevant topics.
- Compliance: Adhere to HIPAA guidelines to maintain confidentiality and protect tenants’ civil rights and property.
- Workplace Policies: Comply with all workplace policies and procedures.
Other Duties:
- Participate in all required training sessions related to the position.
- Perform other duties as assigned by supervisors or management staff.
