What are the responsibilities and job description for the Residential Counselor position at Rehabilitation Support Services, Inc.?
RSS currently has two openings for a Service Plan Writer (Day Time Residence Counselor). One Service Plan Writer will work within an Albany Community Residence and the other will work within a Cohoes Community Residence. These are full time positions and the hours are Monday – Friday 8AM-4:30PM (hours are somewhat flexible).
Minimum qualifications for Service Plan Writer:
- High School diploma or equivalent
- 1 year experience in the Human Services field
- Valid NYS driver's license
Service Plan Writer responsibilities include collaborating with staff and clients on developing service plans according to OMH and Medicaid regulations. The Service Plan Writer will also review weekly contact notes, perform quarterly chart audits, and complete documentation according to agency standards. The ability to type, and to write clearly and objectively is a must. When down staffed, coverage of clients is required.
