Rehabilitation Support Services in Tioga County is seeking a Care Manager. This is a full-time position in Owego, NY.
Minimum qualifications are a Bachelor’s Degree in Human Services or a related field or a CASAC along with 2 years of experience in the Human Services field.
The primary role of a Care Manager is to provide Health Home services to a caseload of 45 individuals with the goal of reducing the rate of hospitalizations through engagement in all necessary community based resources and services, and behavioral and physical health care. The Care Manager will work closely with community providers to ensure appropriate delivery of services to meet the needs of each participant. The Care Manager will conduct assessments to assist in identifying client needs and create individualized plans of care to reflect these needs. Care Managers must be extremely organized and demonstrate an ability to carry out multiple ongoing tasks with the ability to prioritize. This person must be able to work independently and be highly dependable. The Care Manager candidate will also have strong written, verbal and computer skills.
