Rehabilitation Support Services is currently seeking a dedicated and compassionate Transitional and Engagement “Outreach” Specialist to join our team. As a Transitional and Engagement “Outreach” Specialist, you will play pivotal role in connecting individuals with vital services and resources they need to thrive within our behavioral health outpatient clinical setting.
Responsibilities include: -Maintain a caseload to support client engagement, conduct outreach visits to individuals within the community, Advocate for clients to ensure they receive necessary services and supports, Keep detailed records activities, Collaborate with internal and external stakeholders to clients’ need, Stay informed about available community resources, Provide coordination of care, Attend meetings, workshops and training sessions, Report to Deputy Director of Community Services, perform other related duties as assigned.
Qualifications: At least one year of experience in a similar role, preferably in an outpatient or community-based setting, Strong interpersonal and communication skills, Knowledge of local resources and services, Ability to work independently.
