What are the responsibilities and job description for the Wellness Leader position at RehabVisions?
Wellness Leader will assist skilled nursing residents with exercise wellness programs designed by Physical, Occupational and Speech Therapy Staff.

work setting is at a skilled nursing facility in Omaha, Nebraska
-
PRN or part-time hours 20 - 29 hours per week
-
preferred training as a certified nursing assistant (CNA), exercise specialist or a therapy student
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Maintain a current file/list of residents participating in wellness program.

Review resident wellness plans and recommendations from consultants (O.T., P.T., S.T., etc.) and supervise program as outlined.

Transport residents as needed to and from wellness area.

Document correctly on the residents performance, equipment, checklists, incident reports, etc.

Assist residents in completion of wellness program activities on order of physician or therapist.

Ambulate (not gait train) resident per established wellness program.

Reinforce safe transfer techniques to residents and care givers, including use of gait belt as taught by therapist.

Participate in resident care conferences, facility meetings, educational programs, etc. as requested or assigned.

Assist residents on and off exercise equipment utilizing safe transfer techniques.

Assist residents in completion of upper and lower body exercises as outlined in wellness program.

Facilitate completion of wellness programs by assisting and supervising residents to ensure the recommended level of resistance and repetitions are completed.

Maintain confidentiality of patient information in accordance with HIPAA and facility rules.

Perform housekeeping tasks to keep equipment and department clean.

Adhere to safety, risk, and emergency procedures.
EDUCATIONAL REQUIREMENTS

Minimum of high school diploma

Preferred training as certified nursing assistant, exercise specialist or therapy student
