Position Overview:
We are seeking a compassionate and experienced Nutritionist to join our outpatient team at Rejuven Health. This role will involve assessing patients' dietary needs, creating individualized nutrition plans, and providing ongoing support and education to help clients improve their health. The ideal candidate will be skilled in developing treatment plans for a wide range of conditions, including weight management, chronic disease prevention, and overall health optimization.
Key Responsibilities:
- Patient Assessment & Evaluation:
- Conduct initial nutrition assessments for outpatient clients, reviewing medical history, current lifestyle, dietary habits, and health goals.
- Evaluate lab results, medical diagnoses, and other relevant information to guide nutrition recommendations.
- Treatment Plan Development:
- Create personalized nutrition plans based on the assessment findings, addressing specific health conditions, dietary restrictions, and client preferences.
- Provide evidence-based recommendations for improving nutrition and lifestyle choices.
- Educate patients on proper nutrition, meal planning, and healthy eating habits.
- Ongoing Follow-Up & Support:
- Monitor patient progress through regular follow-up sessions, adjusting treatment plans as needed to ensure success and optimal outcomes.
- Offer continued support via one-on-one consultations, phone calls, or email, addressing questions or concerns related to nutrition and lifestyle changes.
- Collaboration with Healthcare Team:
- Work collaboratively with physicians, therapists, and other healthcare professionals to ensure a multidisciplinary approach to patient care.
- Provide nutrition education and support as part of an integrated treatment plan for patients.
- Record Keeping & Documentation:
- Maintain accurate and up-to-date patient records, including assessments, treatment plans, progress notes, and any changes in patient status.
- Ensure compliance with privacy regulations and clinic policies regarding documentation.
- Education & Community Outreach:
- Conduct group nutrition classes or workshops for patients and the community.
- Stay current with the latest nutrition research and trends to provide the best possible care.
Qualifications:
- Bachelor's degree in Nutrition, Dietetics, or a related field (required).
- Registered Dietitian Nutritionist (RDN) certification (preferred).
- At least 2 years of experience as a nutritionist or dietitian, preferably in an outpatient setting.
- Strong knowledge of nutrition science, medical nutrition therapy, and health promotion.
- Excellent communication skills, with the ability to educate and motivate patients.
- Ability to create individualized nutrition plans based on client goals and health conditions.
- Compassionate, patient-centered approach with strong interpersonal skills.
- Ability to work independently and as part of a collaborative healthcare team.
