Position Overview:
We are seeking a licensed and experienced Primary Care Nurse Practitioner (NP) to provide primary healthcare services to a diverse patient population. The ideal candidate is dedicated to delivering exceptional care, fostering strong patient relationships, and contributing to a collaborative and supportive team environment.
Responsibilities:
- Conduct comprehensive patient assessments, including physical exams and health histories.
- Diagnose and treat acute and chronic illnesses, managing patient care plans effectively.
- Prescribe medications and therapies as appropriate within scope of practice.
- Educate patients on health promotion, disease prevention, and lifestyle management.
- Collaborate with other healthcare providers to ensure integrated and holistic care.
- Maintain accurate and timely documentation in the electronic health record (EHR) system.
- Participate in quality improvement initiatives and promote evidence-based practices.
Qualifications:
- Master’s or Doctoral degree in Nursing from an accredited program.
- Current Nurse Practitioner (NP) license in Virginia or applicable state.
- Board certification in Family Practice, Adult-Gerontology, or a related specialty.
- Minimum of 3 years of clinical experience in primary care (preferred).
- Autonomous practice met by VA State requirements
- Strong interpersonal, communication, and organizational skills.
- Proficient in EHR systems and comfortable with technology.
