Job Title: Home Health Aide
General Summary:
The Home Health Aide provides personal care/related services in the home.
Qualifications:
- Ability to read and write English, and have an awareness of various safety and danger/warning signs in regards to home care
- Completion of six months experience working in a hospital, nursing home, certified home care, private duty home care, or as a direct care worker
- Emotional and mental maturity necessary for establishing and maintaining good work relationship with the client, client’s family, and personnel of Relevar Home Care
Licensure Required:
- Valid driver’s license
Responsibilities:
Tasks to be performed must be assigned by and performed under the supervision of an RN who is responsible for the client care.
Duties include:
- Provide companionship and bedside assistance for client on hospice.
- Assist in maintaining a safe environment, including wheel chair and hospital bed safety, routine housecleaning
- Plan and prepare nutritious meals.
- Assist client with ambulation with or without assistive devices
- Follow universal, fall, skin integrity, bleeding, aspiration, oxygen, diabetic. seizure precautions
- Remind and supervise client taking medications
- Attend and/or complete eight in-services per year
- Complete documentation of daily care provided to client
- Report change in client’s mental or physical condition to immediate supervisor
- Confirm weekly schedule
- Maintain confidentiality per agency policy and HIPPA
- Present a positive, highly motivated and enthusiastic image about self and agency
- Promote harmonious work environment
- May include additional High Tech skills designated by RN through appropriate training and competency
Physical Requirements:
- May sit comfortably to perform documentation. Walking; standing; bending to perform job functions
- The ability to drive an automobile, with current state drivers license and valid auto insurance
- Must be able to reach, stoop, bend, lift, and transfer clients
Functional Abilities:
- Must be able to travel to prospective clients' place of residence in own vehicle
- Work requires light physical exertion (up to 30 pounds) on a frequent basis or continuous walking; or moderate physical exertion (up to 50 pounds on a occasional basis (up to 15% of time); or heavy physical exertion (over 50 pounds on a intermittent basis)
- Work may involve exposure to elements and environments that are potentially hazardous or harmful, requiring use of well defined precautions for protection from injury or illness
