MDS Coordinator

Job Description
The MDS Coordinator is responsible for coordinating and completing the Minimum Data Set (MDS) assessments for all residents in a long-term care facility. The MDS Coordinator will work closely with the interdisciplinary team to ensure accurate and timely completion of assessments, as well as ongoing monitoring and updating of resident care plans.
Responsibilities
- Coordinate and complete MDS assessments for all residents in the facility
- Ensure accuracy and completeness of assessments
- Collaborate with the interdisciplinary team to develop and update resident care plans
- Monitor and update resident care plans as needed
- Ensure compliance with all state and federal regulations related to MDS assessments
- Provide education and training to staff on MDS assessments and related topics
- Participate in quality improvement initiatives related to resident care
- Perform other duties as assigned
Requirements
- Current RN or LPN/LVN license in the state of Georgia
- Minimum of 2 years of experience in long-term care
- Knowledge of MDS assessment process and regulations
- Strong organizational and communication skills
- Ability to work independently and as part of a team
- Proficiency in Microsoft Office and electronic medical records
