This opportunity is well-suited for a clinician looking to build their patient load gradually while maintaining flexibility over their own schedule. The position involves providing one-on-one, skilled therapy in the comfort of the patient’s home.
Primary Responsibilities
Preferred Candidate:
We are currently prioritizing candidates who can serve the Northside Chicago neighborhoods, including but not limited to: Uptown, Andersonville, Rogers Park, Edgewater, Lincoln Square, and nearby communities.

Primary Responsibilities
- Conduct in-home physical therapy evaluations and develop individualized treatment plans
- Deliver skilled physical therapy services aimed at improving mobility, strength, balance, and safety
- Educate patients and caregivers on home exercise programs and injury prevention
- Monitor patient progress and adjust care plans accordingly
- Coordinate with physicians, nurses, and care managers to ensure seamless care
- Complete accurate, timely clinical documentation in accordance with state and federal regulations
Preferred Candidate:
- Licensed Physical Therapist in the State of Illinois (required)
- Minimum of 1 year of experience in home health or adult rehab setting preferred
- Working knowledge of electronic documentation systems (e.g., Axxess, Kinnser/WellSky, etc.)
- Strong time management, organizational, and interpersonal skills
- Must be self-motivated, professional, and reliable
- Valid driver’s license, reliable transportation, and current auto insurance are required
