Individual Planning Specialist- Continuous Residential Support (Lisbon)- $21.25/Hr
Closing Date: 03/17/2025
FT 32 Hr/wk Assists program participants in formulating goals related to activities of daily living, community based social and recreational events, employment, education, health, and wellness and/or financial skills and to represent these goals in an Action Plan. Prepares for each program participant all required documentation for semi-annual and annual individual plan meetings in accord with DDS’s Procedure to endure timely documentation. Serves as liaison between programmic, natural, and/or legal support networks to ensure all who wish to attend Individual Plan meetings or processes are informed regarding all relevant meeting details

Hours: Mon - Thurs, 7:00 am - 3:00 pm
Duties and Responsibilities:
- To perform all aspects of the Individual Planning process for residential program participants in North and/or South region.
- To assist program participants in formulating goals related to activities of daily living, community based social and recreational events, employment, education, health & wellness, and/or financial skills and to represent these goals in an Action Plan.
- To collaborate with the Residential Health Care Specialist, the Residential Program Director, the Service Director and the Nurse Consultant to help ensure pertinent medical information is represented clearly in Individual Planning forms and processes.
- To utilize programmic data systems to track behaviors of concern for Individual palnning, Program Review, and Utilization of Resources Review processes.
Education and Experience:
- Associate’s degree in related field or equivalent work experience preferred
- HSD or GED required
- Valid Driver’s License required
- Bilingual Preferred
