Job Summary:
Responsible for overseeing and coordinating all aspects of clinical and administrative operations within our hospice care division in accordance with applicable law and regulations. You will play a crucial role in ensuring compassionate and high-quality patient care, regulatory compliance, and operational efficiency. You will lead a team of healthcare professionals dedicated to providing comfort, dignity, and support to patients and their families during this sensitive time.
Essential Functions:
- Develop and implement strategic plans for the hospice program in alignment with organizational goals.
- Ensure the delivery of high-quality care that meets the physical, emotional, and spiritual needs of patients and families.
- Recruit, train, and supervise a multidisciplinary team of healthcare professionals including nurses, social workers, chaplains, and other support staff.
- Foster a collaborative and supportive team culture that encourages professional growth and development.
- Relationship Building: Establish and maintain strong, positive relationships with referring physicians, medical practices, and other healthcare stakeholders in the community.
- Educational Outreach: Provide information and educational materials to referring physicians about our services, specialties, and the latest advancements in healthcare.
- Identify Opportunities: Continuously identify opportunities for collaboration and partnership with referring physicians to increase patient referrals.
- Problem Resolution: Act as a liaison between referring physicians and our healthcare facility to address any concerns, issues, or feedback, ensuring a smooth referral process.
- Market Analysis: Monitor and analyze market trends, competition, and the healthcare landscape to identify potential growth areas and strategies.
- Data Management: Maintain accurate records of interactions, referral sources, and outcomes to track progress and performance.
- Collaboration: Work closely with our medical team, marketing department, and other relevant departments to develop and implement referral strategies.
- Feedback Collection: Gather feedback from referring physicians to continuously improve our services and meet their needs.
- Compliance: Ensure all interactions and communications with referring physicians adhere to relevant healthcare laws and regulations.
Additional Responsibilities:

- Participates actively in continuing education and in-services.
- Maintains confidentiality of patient information and business trade practices.
- Assumes accountability for reporting incidents and complaints according to Agency policy.

Minimum Position Qualifications:
- Education: Bachelor’s degree in Nursing, Healthcare Administration, Business Administration, or related field.
- Experience: 5 years of experience as a Registered Nurse within the Home Care setting. Minimum of 3 years of progressive experience in home health care management with demonstrated leadership and managerial skills.
- License / Certification: Current State RN license; additional certifications in hospice and palliative care preferred; current CPR; current driver’s license and proof of auto liability.
Environmental Conditions:
Works under a variety of conditions in facilities and offices. Must have the ability to work a flexible schedule and travel locally. Some exposure to unpleasant weather and moderate noise levels. Tasks may involve exposure to bloodborne pathogens. Moderate stress and emotional demands.
Physical Requirements:
Sitting is required. Must be able to handle stressful situations in a calm and courteous manner. Ability to work under pressure to meet deadlines and agency needs. Ability to travel.
