Job Summary :
The PCC assumes the responsibilities of coordinating the office in accordance with state, federal and local regulations. Responsible for data entry of patient information and daily reports. You will oversee administrative support to ensure all departments can function effectively. Your organizational skills and leadership abilities will be instrumental in maintaining a productive and positive workplace atmosphere.
Essential Functions :
- Helps in maintaining office functions and machines and processing clerical information.
- Assist with billing process as needed.
- Answers phones, opens mail, greets visitors in office and tracks physician’s orders.
- Promotes compliance with federal and state regulations. Stays current on all changes in hospice regulations.
- Must present a positive and polite image to customers.
- Data Management : Maintain accurate records of interactions, referral sources, and outcomes to track progress and performance. Ensures all patient visits are scheduled and performed timely.
- Prepares weekly clinicians schedules once approved by manager. Ensures clinicians are assigned and scheduled efficiently with geographic locations, matching skill with required care and optimizing clinicians’ capacity. Works with field clinicians to make certain that all visits are scheduled and completed as ordered.
- Ensures missed visits are rescheduled appropriately or refers to Manager for resolution.
- Manages patient calls and field staff related scheduling issues. Monitors patient in hospital and communicates, ensuring any follow ups as necessary
- Completes all requested scheduling of orders and services such as lab work, supervisor visits, addon visits and disciplines as required.
- Works closely with Clinical Manager to ensure timely communication with patients, clinicians, referral sources and other office personnel.
- Runs all necessary reports and responds to workflow by taking appropriate actions.
- Assist with external and internal transfers of patients between care centers or Hospice services.
- Helps with supply closet to assure that all supplies are maintained and are within the expiration dates and are packaged appropriately and to serve as liaison to field personnel for patient supply needs.

Works in a routine office environment. Ability to work flexible schedule and some extended hours, ability to travel locally, some exposure to unpleasant weather. Noise level may be moderately high.
Physical Requirements : Prolonged sitting and standing are required. Occasional need to lift, pull, carry or push items weighing up to 50 lbs. Frequent stooping, kneeling, and reaching for items. Requires ability to handle stressful situations in a calm and courteous manner. Requires working under some stressful conditions to meet deadlines and agency needs. Requires Excellent problem-solving skills.
