Remedi SeniorCare is currently looking for enthusiastic, motivated and talented Clinical Pharmacist to join its Consulting team.
As a Consultant Clinical Pharmacist, you will play a crucial role in ensuring the safe and effective use of medications for residents. Medication reviews, collaboration with interdisciplinary teams, monitoring drug therapy, education of staff/residents, and implementation of medication management optimization promotes positive resident outcomes while minimizing risk are also some of your responsibilities as a Consultant Clinical Pharmacists.

This position requires onsite visits to Skilled Nursing customers at least once monthly and Assisted Living customers as least once quarterly. Frequency of these visits may increase depending on acuity of the customer, customer needs, and customer requests.

What You Will Do
- Perform medication regimen reviews of institutional clients in assigned facilities in accordance with applicable state and federal requirements.
- Establish business relationships with key personnel in assigned facilities.
- Perform medication storage inspections and assist with the appropriate disposal of controlled substances where applicable.
- Provide education and industry updates to facility staff.
- Observe medication pass of facility nurses where applicable.
- Perform any other duty or function as required by state of federal long term care regulations or pharmacy requirements.
- Active participation in facility’s Quality Assurance and Performance Improvement meetings and assist in providing pharmacy related data upon request.
- Attendance in additional meetings when mutually agreed upon by pharmacy and facility.
- Maintain thorough and appropriate documentation in the client record/consulting software of all clinical activities and communicates to customer in accordance with pharmacy policy.
- Oversight of facility and prescriber participation in automatic therapeutic interchange program where applicable.
- Maintain clinical skills and pharmacy knowledge through continued education.
- Adhere to Consultant Pharmacist department’s policy and procedures.
- Active collaboration with facility assigned Account Manager.
- Perform staff pharmacist duties as need arises.
What you will bring:
- BS Pharmacy Degree or Doctor of Pharmacy Degree from an accredited college. Certification in Geriatric Pharmacy is preferred, but not required.
- Current registration and licensure in good standing as a Pharmacist in the state (s) where assigned facilities are located.
- Four years performing licensed pharmacist duties.
- Experience is preferably in an institutional pharmacy setting, but is not required.
