About the Role :
As a Certified Home Health Aide (CHHA), you'll be the heart of our mission, providing essential care and support to patients in the comfort of their homes. Your role will promote patient independence, dignity, and comfort—all while ensuring the highest quality of care. This is your chance to truly make a difference !
What You'll Be Doing :
- Measuring and documenting vital signs with every visit to track your patient's progress.
- Assisting with personal hygiene (bathing, grooming, oral care), making sure your patients feel clean, confident, and comfortable.
- Supporting mobility with transfers, ambulation, and prescribed exercises—helping your patients stay on the move!
- Preparing nutritious meals (including therapeutic diets) and assisting with feeding.
- Offering medication reminders (without administering medications).
- Performing light housekeeping, changing linens, and keeping the home environment tidy and safe.
- Observing and reporting changes in patient condition to the supervising nurse or therapist.
- Following personalized care plans, providing care that's tailored to each patient's unique needs.
