What are the responsibilities and job description for the Behavioral Health Counselor position at ReMind Health?
ReMind Health is a psychiatric provider group focused on geriatric patients needing psychiatry located in nursing homes, assisted living, and skilled nursing facilities. Our aim is to provide mental health services to a segment of the community that is often “forgotten” during their rehabilitation and journey back to baseline health. Many of these patients will be discharging to their homes within several weeks. These patients require mental health support during these often difficult transitional times adapting to their new medical diagnosis. Most recently, through our partnership with Villa Maria Recovery, Remind provider and CNO William O'hara, PMHNP, has become the clinical director for their substance abuse program and will be providing intensive behavioral support to this patient population.
GOALS:
- Support the patient while they transition to their new baseline
- Provide accurate and timely charting to the facility
- Provide psychotherapy and comprehensive psychiatric evaluations

About you:
- Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapy (LMFT), PsyD with Colorado state license. Fully independent, we are unable to take trainees at this time.
