ESSENTIAL DUTIES AND RESPONSIBILITIES FOR A PHYSICAL THERAPIST (PT) This summarizes the major essential responsibilities and duties regularly performed and normally assigned to this position. Listed below are a summary of the position responsibilities. Other duties may be assigned.
Supervise Physical Therapy Assistants, Physical Therapists with temporary licenses, students and ancillary staff in accordance with state licensure requirements and professional standards.
Performs (including documentation of such) screens, evaluations, progress reports, treatment encounter notes and reevaluations of residents’ in regards to physical therapy needs upon referral in a timely manner.
Establishes long term and short term goals determined by evaluation in conjunction with resident, physician, caregiver and additional disciplines involved with patient care
Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
Responsible for the effective and efficient delivery and documentation of rehab services as outlined in the residents’ plan of care.
Fully participates in patient care, staffing and other related meetings as assigned by DOR.
Monitors and makes changes to the residents’ plan of care as needed
Promotes safety for residents and employees in all areas of the facility.
Places residents’ needs first, to ensure that residents’ and families receive excellent clinical care in a caring and compassionate manner.
Orders, cleans, maintains, and reports issues with, supplies and equipment as needed.
Establish professional relationships with residents, family members and facility team to ensure that consistent communication is provided to them on a regular basis
Partner with residents, family members and facility staff through the care and discharge planning process to facilitate transition to the appropriate level of care.
Assist as needed with facility training(s).
Performs other duties as assigned.
EDUCATION / VOCATIONAL REQUIREMENTS FOR PHYSICAL THERAPIST (PT) :
Graduate of a school of Physical Therapy approved by the Commission on Accreditation in Physical Therapy Education (CAPTE)
Valid Physical Therapy license and able to practice in the state(s) of licensure.
EXPERIENCE : A minimum of 1 year experience in long term care setting preferred, however, not required.
