Description
We are a growing, fast-paced physician-led office that is a leader in our field of treating kidney disease looking for a full-time Advanced Practice Provider to join our team. The right candidate will thrive in a multi-tasking environment serving our patients and serve as a partner with our team of 29 excellent physicians and 12 Advanced Practice Providers. As an Advanced Practice Provider within RenalCare Associates, you will play a crucial role in providing comprehensive care to a subset of dialysis patients as well as our CKD patients. APPs in this position round on the same ESRD patients each month and enjoy building a rapport and relationship with those patients while educating, empowering, and improving patient outcomes in the ESRD population. You will also play a key role in our CKD patient population while managing their CKD and related conditions, empowering them with education, aiming for increased quality of life, prolonged disease progression, hospitalization reduction, optimal start of dialysis, and an increase in transplant and home dialysis patients. We are partnered with Evergreen Nephrology and their interdisciplinary teams to support our managed care approach with our patients. The majority of time spent in this role will reside in our CKD offices and assigned dialysis unit (including some travel to satellite offices).This is an exciting opportunity to make a positive impact on the lives of patients and contribute to the success of our nephrology practice.

Requirements
Completion of an accredited program for Nurse Practitioner (NP). Nephrology experience preferred but not required.

Salary : $110,000 - $130,000
