The Transitional Living Specialist at Renewed Vision CDDC will be responsible for supporting individuals in their transition from institutional or temporary living arrangements to independent, sustainable living. This role involves providing guidance, resources, and emotional support to help individuals build the skills and confidence needed to thrive in a new living environment. The Specialist will develop individualized plans, coordinate services, and offer ongoing support to ensure successful transitions and long-term stability.
Key Responsibilities:
- Conduct assessments to evaluate the needs and goals of individuals transitioning to independent living.
- Develop and implement individualized transition plans, including housing, employment, education, and life skills.
- Provide case management, advocacy, and emotional support to individuals during the transition process.
- Collaborate with local service providers, community resources, and housing programs to secure appropriate living arrangements.
- Facilitate life skills training on topics such as budgeting, cooking, job readiness, and self-care.
- Monitor the progress of individuals and adjust plans as needed to ensure success and address challenges.
- Maintain detailed records of services provided, progress, and any incidents or concerns.
- Provide crisis intervention as needed and work to ensure the safety and well-being of all individuals served.
- Participate in team meetings and contribute to program development and improvement initiatives.
Qualifications:
- Bachelor’s degree in Social Work, Psychology, or a related field (or equivalent experience).
- Minimum 2 years experience working with individuals in transitional living, homelessness, or similar settings.
- Strong knowledge of community resources and services for individuals in transition.
- Excellent communication, problem-solving, and organizational and time management skills.
- Proficiency in case documentation, report writing and data management systems.
- Ability to work independently and as part of a team.
- Crisis intervention and problem solving abilities.
- Compassionate, patient, and dedicated to helping individuals achieve independence and self-sufficiency.
- Must pass a comprehensive background check. Need a valid drivers license and acceptable driving record.
Working Conditions:
- Full-time position with occasional evening or weekend hours.
- Occasional travel may be required for outreach and collaboration with partner organizations.
