JOB TITLE: Wellness Visit Coordinator/Float Nurse
DEPARTMENT: Belleville Medical Clinic - Nursing
JOB RELATIONSHIPS:
Reports:
Responsible To: Clinic Nurse Manager
Responsible For: Does not supervise other employees
Interrelationships: Works cooperatively with clinic and hospital departments and medical staff, employees, patients and visitors.
JOB SUMMARY:
The Wellness Visit Coordinator will provide direct and indirect care activities under direct supervision. This individual will be responsible for determining who is eligible for Medicare Wellness Visits, scheduling these visits, and assuring needed documentation is complete prior to the visit.
Float duties will include but not limited to placing patients in rooms; to assist with procedures, treatments and examinations, under direction of a registered nurse or licensed physician; recording vital statistics, obtaining nature of reason presenting for office visit, complete basic medical record tasks;including entering patient medical, social history, current medications, maintaining adequate supplies and stocking exam rooms.
JOB QUALIFICATIONS:
Experience:
- Previous healthcare experience in a similar or related healthcare position, preferred.
Education:
- Must be 18 years of age or older with a high school diploma or equivalent.
- CPR Certification
- CCMA, must be obtained and maintained upon employment.
KEY COMPETENCIES:
- Excellent written and verbal communication.
- Knowledge in MS Office, Gmail, and patient management software.
- Strong organizational, planning, time management and prioritizing skills.
- Ability to multitask.

JOB DUTIES:
- Determine who is eligible for Medicare Annual Wellness visits and contact patients prior to appointment to obtain needed documentation for visit.
- Basic knowledge of medical software/computer.
- Make appointments, as needed.
- Prepare clean and organized treatment or examination patient rooms.
- Assist with examination of patients, under the supervision of a physician.
- Assist with treatment of patients, under the supervision of a physician.
- Perform select clinical duties.
- Vitals/Blood pressure check.
- Complete and enter patient medical, social history and current medications into the EMR.
- Document communication with patients in charts.
- Perform basic clerical duties including answering the phone, maintaining records,and filing.
- Maintain patient confidentiality at all times.
- Maintain and stock patient room supplies.
WORKING CONDITIONS:
- Occasionally exposed to the risk of blood borne diseases.
- Contact with patients under a wide variety of circumstances.
- Occasionally exposed to unpleasant elements (accidents, injuries and illness).
- Subject to varying and unpredictable situations.

ESSENTIAL FUNCTIONS:
- Stand, walk, bend, stoop and stretch for prolonged periods of time.
- Push/pull patients in wheelchairs (up to approximately 250 pounds).
- Assist patients on and off the procedure table.
- Daily lifts supplies/equipment.
