We are seeking a compassionate and skilled RN to join our Hospice Admissions team. In this role, you will support patients and their families by providing direct care and coordinating with the interdisciplinary team to ensure an optimal care experience. Responsibilities:

• Develop and implement patient plans of care that meet the unique needs of each patient
• Collaborate with the interdisciplinary team to ensure seamless coordination of care
• Provide education and support to patients and their families to promote self-care and independence
• Maintain accurate and up-to-date clinical records and reports
• Participate in quality improvement initiatives to enhance the delivery of care
• Ensure compliance with all state and federal legal and regulatory requirements

Qualifications:

• Current RN licensure in the state of operation
• 1 year nursing experience required, 3 years preferred
• Experience in hospice or a similar setting preferred
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
