About the Role:
We are seeking a compassionate and skilled Licensed Practical Nurse or Licensed Vocational Nurse to provide high-quality patient care in our community living facilities.
Key Responsibilities:
- Provide 1:1 care for clients, following their individualized plans of care while maintaining their dignity and independence.
- Monitor clients' vital statistics and report any abnormal findings to the case manager and physician.
- Administer medications, feedings, oxygen, and ostomy care as needed, according to the client's plan of care.
- Complete established routines with other care professionals, such as physical therapists, occupational therapists, and speech therapists.
- Ensure infection control policies are maintained at all times and report any possible communicable diseases or infections.
- Accurately document nursing actions and communicate with family members, case managers, and physicians.
- Order supplies and medications as needed to ensure adequate amounts are available.
- Monitor incidents involving individuals and complete incident reports timely and accurately.
- Ensure medical equipment is maintained and report faulty equipment for repair.
Requirements:
- Candidate must have completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.
- Knowledge of individual service plans, including understanding of client rights and health needs.
- Proof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice.
- CPR certification.
- Excellent interpersonal skills, including customer service, patience, empathy, and compassion.
- In-depth understanding of patient care policies and regulations.
- Familiarity with symptoms and signs of improvement for several medical conditions.
