Connections to Care Case Manager
The Rescue Mission is seeking a Connections to Care Case Manager to work in our Emergency Shelter located in Syracuse, NY. This position assists clients by assessing Substance Use Disorder needs and works with provider networks to identify addiction treatment, harm reduction, and recovery services that match client goals. This position will assist in maintaining a structured, safe, and supportive environment which reflects the mission, vision and values of the Rescue Mission.
Duties & Responsibilities:
The Connections to Care Case Manager will manage a caseload of clients, ensuring accurate and timely documentation while conducting substance use disorder assessments to identify client strengths and challenges. This position will provide supportive counseling, encouraging clients towards positive growth, and assist them in identifying treatment, harm reduction, and recovery services. Regular monitoring of client progress, coordination with internal and external service providers, and fostering relationships with community partners are key aspects of this role. This position will ensure confidentiality and maintain professional boundaries, collaborate with community services including mental health, addiction, legal, and financial support, and respond to communication from partner agencies within three days. Attendance at required meetings and events, adherence to safety policies, and maintaining regular in-person attendance are essential.
Requirements:
- Bachelor’s degree in social work or related field preferred.
- Associate’s degree in human services and CASAC training eligible for consideration.
- Valid NYS Driver License in compliance with company driving criteria
- Familiarity with community services a plus
- Bi-lingual skills a plus
- Must follow CDC Guidance, including but not limited to, wearing a mask when working.
- Full vaccination including boosted (when eligible) against Covid-19 is strongly encouraged.

Rate of Pay: $23.00- $25.00 per hour
