Case Manager
The role of a Case Manager is to guide guests in moving from homelessness to permanent housing by providing direct case management services and following a Trauma Informed approach. This position is responsible for working one-on-one with each guest/household to identify their obstacles to permanent housing and to develop action plans to address and overcome those barriers to achieving housing stability.
Responsibilities:
- Create individualized housing plans in collaboration with guests.
- Meet with guests regularly to discuss housing plan goals.
- Assist guests in accessing housing resources.
- Assist guest in accessing community resources, entitlement benefits, employment resources, and healthcare.
- Continually promote self-determination and self-sufficiency in interactions with guests.
- Completed case notes on face to face/telephone/email contact with guest or collateral contact before the end of each shift.
- Compile all necessary data and statistics for reporting purposes.
- Distribute referrals as needed and check with referral agencies for follow up information when needed. Referrals may include, but are not limited to resources for housing, childcare, tangible goods, financial assistance, health care, employment programs, behavioral health services, substance abuse services, etc.
- Work in collaboration with programs and all other agency staff to facilitate team environment.
- Enter, update, and maintain data in HMIS.
- Maintain guest files accordingly.
- Present cases at case staff meetings for input from other staff members and commit to group decisions.
- Utilize Motivational Interviewing, person centered planning, a recovery-oriented framework, and housing first when working with guests.
- Complete and maintain ongoing training, CEUs, or other required certifications as needed.
- Other duties as assigned by management.
Qualifications:
- Bachelor’s degree in Human Services, Social Work, or related filed preferred or two years’ experience working in a human services environment; and,
- Experience working with the homeless population is preferred.
- Bilingual Preferred

- Ability to articulate the mission, programs, and services of Rescue Outreach Mission of Central Florida, Inc.
Job Types: Full-time, Part-time
Pay: $18.00 - $23.00 per hour
