Job Description

As a home health registered nurse case manager, you will lead and collaborate with our team-based environment to provide management and delivery of patient care plans. This includes:

• Providing high-quality patient care through effective management and delivery of patient care plans.
• Collaborating with our team-based environment to ensure seamless patient care.
• Communicating effectively with patients, families, and healthcare providers to ensure comprehensive care.

Requirements

To be successful in this role, you will need:

• Graduation from an approved school of practical nursing.
• One year of RN experience and the clinical competence to deliver quality patient care.
• Demonstrated experience with tablets, mobile phones, and EMR software.
