Responsibilities
This position supports patients in Glenview, Grayslake and surrounding areas. The ideal candidate will have:
- A Master's in Social Work from a graduate school accredited by the Council of Social Work.
- One (1) to two (2) years post graduate social work experience in a health care setting.
- A valid driver's license and ability to spend ~20% of your day driving to/from patient locations.
- The skills needed to self-manage your time and schedule.
- Demonstrated experience with tablets, mobile phones and EMR software.

Requirements
To be successful in this role, you must hold, in good standing, a license as a Medical Social Worker issued by the state in which you work. A commitment to consistently meet critical deadlines for charting is also required.
