Residential Home Health and Hospice is looking for an experienced Nurse Case Manager to join our care team. As a home health registered nurse, you will lead the delivery of patient care plans in partnership with our dedicated team.

To excel in this role, you should possess a RN license in the state you work, graduate from an approved school of practical nursing, and have one year of RN experience with clinical competence. You must also be able to self-manage your time and schedule, demonstrate experience with tablets, mobile phones, and EMR software, and maintain a commitment to meeting critical deadlines for charting.
In addition to these qualifications, we require you to be able to spend approximately 20% of your day driving to/from patient locations and have a valid driver's license.
