This position supports patients in Lakeland and surrounding areas, working closely with our dedicated team to provide high-quality patient care.
Responsibilities
- Lead and collaborate with the team to develop and implement patient care plans
- Manage and coordinate patient care services to ensure optimal outcomes
- Communicate effectively with patients, families, and healthcare professionals
- Collaborate with multidisciplinary teams to address patient needs and concerns
- Document patient information accurately and efficiently using state-of-the-art technology
- Participate in ongoing education and training to enhance clinical skills and knowledge
Requirements
- RN license in the state of employment
- Graduate from an approved school of practical nursing
- One year of RN experience and demonstrated clinical competence
- Current driver's license and ability to spend approximately 20% of work time driving to/from patient locations
- A commitment to meeting critical deadlines for charting and self-managing time and schedule
- Demonstrated experience with tablets, mobile phones, and EMR software
