Job Responsibilities
- Obtain complete and accurate demographic information, medical history, and primary care physician information for each patient
- Introduce our services to patients and caregivers, explaining the scope of our care and coordinating start-of-care visits
- Provide community education as requested by sales teams, including educational in-services, blood pressure clinics, and health fairs
- Deliver patient education at bedside and arrange post-discharge physician follow-up appointments
Requirements:
- Previous Home Health experience required
- Minimum of one year of experience in an acute care setting
