About the Role
This is a part-time weekend position, working Friday to Sunday, 8am to 8pm. You will be responsible for providing direct patient care, including administering medications, wound care, and vital sign monitoring. Our patients are in need of supportive care and comfort during this challenging time, and we rely on our LPNs to make a meaningful difference in their lives.
Responsibilities
* Provide direct patient care, including medication administration, wound care, and vital sign monitoring* Collaborate with the interdisciplinary team to develop and implement individualized care plans* Perform routine medical procedures, such as injections and blood draws* Document patient information accurately and completely* Communicate effectively with patients, families, and healthcare teams
Requirements
* Current LPN license in the state where you work* Graduate from an approved school of practical nursing* Two years of LPN experience, with hospice experience preferred* Ability to work independently and as part of a team* Strong communication and interpersonal skills* Familiarity with electronic medical records software* Reliable transportation and ability to travel to patient locations as needed
