We Are Seeking:

• Compassionate Home Health Medical Social Workers with a Master's degree in Social Work from a graduate school accredited by the Council on Social Work Education.
• One to two years of post-graduate social work experience in a healthcare setting.
• Must hold a license as a Medical Social Worker issued by the state in which they work.
• A valid driver's license and ability to spend 20% of their day driving to/from patient locations
• A commitment to meeting critical deadlines for charting
• Self-management skills and time management
• Experience with tablets, mobile phones, and EMR software
