Key Responsibilities
- Obtaining accurate demographic information, medical history, and primary care physician details for each patient.
- Introducing our services to patients and caregivers, explaining the scope of our services and coordinating start-of-care visits.
- Conducting community education sessions, including educational in-services, blood pressure clinics, and health fairs.
- Patient education at bedside, arranging post-discharge physician follow-up appointments, and sharing important discharge information with primary care physicians.
Requirements
- Previous home health experience is essential.
- A minimum of one year of experience in an acute care setting is required.
