Responsibilities
This position supports patients in Traverse City and surrounding areas. As a home health registered nurse case manager, you will lead and collaborate with our team to provide management and delivery of patient care plans. This role requires a commitment to consistently meet critical deadlines for charting and self-manage your time and schedule.
Requirements
- RN license in the state you work
- Graduate from an approved school of practical nursing
- One year of RN experience and the clinical competence to deliver quality patient care
- Current driver's license and ability to spend ~20% of your day driving to/from patient locations
- Demonstrated experience with tablets, mobile phones, and EMR software
