Job Description
This is a role on a team for a Geriatric or Primary Care Nurse Practitioner located in Northern Virginia, VA. Patient population ages 65 through 100 . On-site visits in the counties of Arlington, Fairfax, and Loudon. The role involves a holistic care focus, encompassing the diagnosis and management of acute and chronic conditions, routine check-ups, medication prescription, and the development of treatment plans tailored to the geriatric population. The Nurse Practitioner will collaborate with physicians and other healthcare professionals, educate patients and their families about health management, and ensure compliance with healthcare regulations. Some televisit work from home is acceptable as part of this role. Compensation model per case rate with a quarterly performance revenue bonus.
Prerequisites
- Autonomous Practice Authority in the State of Virginia
- Current Nurse Practitioner license
- Doctorate or Master's degree in Nursing
- Previous experience in geriatric care or adult primary care working with aging populations is a requirement
- Self-starter, results-driven, independent
- Holistic and preventive care philosophy
- Passionate and care-focused for aging individuals
- Strong collaboration and communication skills to work with families, care managers, ancillary therapists, Home Health nurses, facility healthcare staff, and specialty care providers
- Adept at identifying deficits in health management and guiding patients and their families about health, disease management, care environments, and end-of-life care
- Experienced in diagnosing and managing acute and co-morbid chronic conditions in geriatric patients
- Proficiency in understanding poly-pharmacy in the elderly and safe prescribing practices
- Knowledge of healthcare regulations and compliance
- DEA License
- Independent Malpractice Insurance
Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans.
- Utilize EHR systems for accurate documentation. patient tracking, and billing.
- Collaborate with healthcare teams to provide primary care and manage chronic conditions.
- Educate patients and families on health management and preventive care strategies.
- Stay current with clinical practices and participate in ongoing professional development.
