We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our healthcare team. Responsibilities:
- Conduct comprehensive assessments of patients' medical conditions, treatment plans, order all medications as needed.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers
- Coordinate and facilitate communication between patients, families, healthcare providers, and payers to ensure continuity of care
- Monitor patient progress and make necessary adjustments to care plans as needed
- Provide education and support to patients and their families regarding disease management, treatment options, and available resources
- Utilize utilization management strategies to optimize resource allocation and ensure appropriate levels of care
- Collaborate with healthcare team members to ensure effective coordination of services across the continuum of care

Qualifications:
