```Duties```
- Provide compassionate and skilled nursing care to patients in a hospice setting
- Assess and monitor patients' physical and emotional well-being
- Administer medications and treatments as prescribed by the physician
- Assist with pain management and symptom control
- Collaborate with the interdisciplinary team to develop and implement individualized care plans
- Educate patients and their families on disease management, end-of-life care, and available resources
- Coordinate and communicate with other healthcare professionals involved in the patient's care
- Maintain accurate and up-to-date medical records
```Experience```
- Minimum of 2 years of nursing experience, preferably in hospice or palliative care
- Experience in case management and coordinating patient care
- Proficient in medication administration, including IV therapy and pain management
- Familiarity with aseptic technique and infection control practices
- Knowledge of electronic health record systems (e.g., Athenahealth)
- Ability to provide health coaching and support to patients and their families
