About Our Care Connector Position:

The Care Connector is a vital role that provides support services to participants and uses their unique, lived Resiliency-Recovery Building experience to guide and support others who are recovering from mental health challenges and/or substance use. The Care Connector works collaboratively and in conjunction with our clinical teams to provide support throughout each participant's experience at Resilient Health.

What a Care Connector Does at Resilient Health:

• Exudes characteristics of compassion, empathy, and collaboration for connection with participants, family members and staff.
• Focus on identifying Social Determinants of Health needs of participants.
• Resource specialist to resolve Social Determinants of Health.
• Provides pre-engagement, engagement, and re-engagement of participants, through telephone, in person, telehealth and emails.
• Supports targeted intervention from program staff.
• Assists in follow up contacts, outreach, and updates.
• Provides staff training and education.
• Provides support in Integrated Health Clinics, in client (or family) homes and in the community, as well as in our Resilient Health locations.
• Connects with participant, family member and teams.
• Assists participants in determining the objectives needed to reach their recovery goals.
• Instills hope in individuals by being a role model of recovery.
• Models effective coping techniques and self-help strategies.
• Provides and advocates for effective recovery-based, person-centered services.

Requirements for This Position:

• Arizona Peer Support Specialist Certification required
  
• High school diploma or equivalent required
  
• Has received, or is currently receiving behavioral health services
• Six months experience with clients in a behavioral health setting preferred
• Able to work Monday through Friday 7:00am to 3:00pm
  
• Valid driver's license
• Valid CPR/First Aid certifications or ability to obtain within 30 days of hire.
• Valid Fingerprint Clearance Card or ability to obtain within 90 days of hire.
• Three-year Motor Vehicle Record that meets our auto insurance carrier's coverage requirements.

What This Position Pays: $17.88 - $21.42, depending on education and experience
