The Day Treatment Services Team Member provides clients who attend Day Treatment Services with group and individualized rehabilitation, recreation, education, work and community–based activities to allow clients to attain the highest quality of life, community inclusion, and independence possible given the nature of their injury
1. Interacts with the client to promote and encourage cooperation, self–esteem, self–direction, independence, and normalization.
2. Takes responsible action to ensure health, safety and well being of a client. Ensures privacy and protection from moral, social and financial abuse or exploitation.
3. Participates in assessing the client’s strengths, needs, and preferences in developing her / his plan; implements plan and documents services received and progress.
4. Provides direct client care services when needed including personal care & hygiene, toileting, feeding, grooming, self–care, walking, and basic activities of daily living.
5. Plans and implements age, gender and cultural appropriate activities for both facility-based programming and community-based activities.
6. Plans, develops, and implements individual and / or group sessions. This includes planning session content and curriculum, gathering information and materials for activities, and utilizing techniques that actively engage clients in the learning process and promote clients’ independence.
7. Plans, implements, and supervises clients on community outings and activities and at work by planning the activities, gathering information and materials for the community–based activities, and actively engaging clients in the community. Uses techniques to elicit feedback from clients on specific community goals.
8. Responsible for all job coaching related activities, including supervising the client when working at all times, including during all breaks and transition periods.
9. Responsible for the learning the required job tasks and providing on–the–job training to clients working in the work services programs to ensure the client’s mastery of the required job tasks.
10. Responsible for reinforcing competitive work habits and employability skills with clients at all times, including the dress code, punctuality, and interpersonal behavioral skills.
11. Responsible for maintaining the production and quality standards at the work site, including carrying out job tasks and responsibilities in conjunction with the clients as necessary.
12. Completes all required daily documentation accurately and thoroughly.
13. Dispenses and administers medications as prescribed by the client’s physician by following all company medication policies, procedures, and training protocols for giving, taking, applying, and documenting medications.
14. Communicates and documents any changes in a clients’ status to the Manager, Day Treatment Services.
15. Knows and follows established emergency procedures and participates in emergency drills.
16. Actively works to promote teamwork with a spirit of cooperation within the team and with other teams in serving clients and solving team and individual problems pertaining to assignments, schedules, and performance.
17. Maintains confidentiality of clients’ Protected Health Information. Maintains confidentiality of information pertaining to Resilire employees. Does not engage in gossip or inappropriate discussions about clients, co–workers, or company business.
18. Communicates clients’ progress and needs in a confidential, ethical and timely manner, with the Manager, Day Treatment Services, co–workers and families.
19. Participates in maintenance of Resilire facility by keeping all areas clean, neat, and sanitary and reporting maintenance needs to the Program Manager.
20. Practices infection control as directed by the Department of Public Health, as well as Universal Precautions as specified by the Center for Disease Control. Maintains personal hygiene and grooming at an acceptable level.
21. Operates company and personal vehicles in a safe manner when transporting clients to and from activities in the community and other outings. Operates wheelchair lifts and performs proper wheelchair tie-downs in vehicles according to safe and acceptable procedures.
22. Assists with training new team members as directed.
23. Works flexible schedule and hours as assigned since job is designed around clients’ quality of life which does not necessarily fit into “normal” shift times.
24. Works appropriately with the clients who may exhibit aggressive or difficult behaviors following STRS Behavior Management Policy and training.
25. Provides follow through of therapeutic and / or vocational and / or medical programs which may include, but are not limited to: wheelchair sitting programs, positioning, range of motion exercises, feeding plans, behavior modification plans, bowel and bladder training programs, work training and other individual–specific programs as assigned.
