Patient Care Coordinator
Job Summary :
The Patient Care Coordinator is responsible for maintaining all patient records after admission. Also responsible for communicating needs of callers and transferring to appropriate departments as necessary.
Duties & Responsibilities
- Respond to incoming phone calls and transfer as needed to appropriate departments.
- Answering incoming calls to handle referrals, scheduling and coordination of details to support staff.
- Order pickup of DME for discharged patients.
- Order nursing, aide, and office supplies.
- Keep updated consent and admit packets ready and available for nurse / liaison pickup.
- Assist team in keeping facility binders and home folders up to date.
- Maintains the confidentiality of patient and organization at all times.
- Uploaded any new patient records and attachments to HCHB chart.
- Performs other specific projects relating to data entry and computer operations as required.
