Description
35 Hour work week

Job Duties :
Work as part of a team to provide case management to individuals receiving services in the community through OPWDD funding (Self Direction program) .
Support individuals with disabilities to utilize their services and funds to live in the community.
Communicate (primarily via phone, email or zoom) with the individuals and their support team, assisting them to utilize their budget according to OPWDD rules and regulations.
Obtain and maintain required documentation. Review all documentation for accuracy and guidelines adherence.
Document all communications.
Education :
A Bachelor or Master of Social Work degree is preferred.
A college degree in Human Services or related field will be considered.
A high school diploma or equivalent is required.
Experience providing OPWDD funded services or other case management services may be considered in lieu of higher education.
Knowledge, Skills & Abilities :
2 years of Case Management experience required. Higher education will be considered in lieu of experience.
Strong customer service and computer skills.
Ability to work in a fast-paced environment.
Knowledge of Microsoft Excel, Word, and Outlook.
Excellent verbal and written communication skills.
Strong organizational skills and attention to detail.
