This position will mainly serve the north central region, including Aitkin, Crow Wing, Cass, Wadena, Todd and Morrison counties. The staff hired will need to live within one of the counties listed. Travel throughout these north central region will be frequent. The staff will also be required to drive to the Resource office several times a month. The office is located in Sartell, MN.

Summary/Objective
The Guardian has primary responsibility to act as a legal representative for vulnerable adults. As a guardian, you are responsible for the personal affairs of the individual under guardianship as the courts direct to ensure that appropriate provisions are made for their care, comfort and needs. Communication with the court, county, care providers and other members of the individual’s care team is crucial to make decisions that are in the best interest of the individuals served.
Essential Functions
- Monitor the living situation of the person subject to guardianship to determine the place of abode and appropriateness of the living arrangements.
- Coordinate the person subject to guardianship’s personal affairs (medical care, nutrition, clothing shelter, residence, and safety).
- Person centered care will be paramount in the delivery of services. Adherence to Minnesota’s Olmstead Plan and all subsequent revisions will ensure the individual is living, learning, working and enjoying life in the most integrated setting.
- Adhere to the Guardian’s Code of Ethics.
- Maintain a working knowledge of the services, providers, facilities and funding sources available in the community. Treat all professionals and service providers with respect and courtesy to enhance the working relationship between the provider, person and the guardian to provide the most benefit to the individual.
- Timely and professional communication with the court, county, care providers and other members of the individual’s care team.
- A minimum of two face-to-face contacts with the person will be scheduled every 12 months. Face-to-face contacts will occur more frequently when a case is initially set up and during any transitional periods.
- Maintain proactive communication and engage in continuous process improvement efforts.
- Provide clear documentation of each case and submit necessary monthly, quarterly and annual reports.
- Attend ongoing training, professional development offerings and internal/external meetings as required.

Desired Education and Experience
- Four-year degree in Social Services, Social Work, Psychology or a related field. Comparable experience in a related field may also fulfill this requirement.
- Experience working with vulnerable adults. Previous guardianship experience preferred.
- Successful completion of a background check.
- A valid driver's license, acceptable driving record and proof of auto insurance is required.
Position Type / Expected Hours of Work
This full-time, salaried position. Hours of work are generally Monday through Friday, 8:00 a.m. to 4:30 p.m., but occasional work outside typical work hours, and travel throughout the region is expected.
Environment & Physical Demands
This job operates in a hybrid office environment allowing the flexibility to work remotely but still reside in or near the counties being serviced, with the requirement to be in the office several days a month. This role routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets. This is largely a sedentary role; however, some physical activity is expected. This would require the ability to move and lift 30 lbs., stand for extended periods, and carry items a short distance as necessary.

Salary and Benefits
Competitive salary and benefits, including PERA and a flexible work environment. Starting salary is for a caseload of 30-34 clients. Eligible for a single health insurance policy only, with premiums fully paid for by Resource.
