GENERAL DESCRIPTION / PURPOSE
The Aftercare Case Manager is responsible for ensuring a seamless transition for children discharged from Respite Care of San Antonio (RCSA) to their community placements. This role provides comprehensive support to children, families, and caregivers for six months post-discharge, helping them navigate resources, access services, and maintain stability. The Aftercare Case Manager will collaborate with internal and external stakeholders to develop and implement individualized aftercare plans that align with RCSA’s trauma-informed, child-centered approach and the Texas Child-Centered Care (T3C) framework.
JOB RESPONSIBILITIES
Transition Planning
- Develop child-centered aftercare plans in collaboration with the child, family, caregivers, and treatment team before discharge.
- Ensure all educational, medical, therapeutic, and housing needs are addressed in the aftercare plan, including referrals to community resources.
- Coordinate with schools, healthcare providers, and other community organizations to establish continuity of care and support for the child’s transition.

Crisis Intervention and Advocacy
Documentation and Reporting
Collaboration and Continuous Improvement
REQUIRED QUALIFICATIONS
Must be 21 years of age or older and have favorable findings on background checks to include the Texas Department of Public Safety, Texas Department of Health, and Texas Department of Family and Protective Services criminal record checks and negative drug test and negative TB test.
Bachelor's degree from a four-year college or university required in Social Work, Psychology, Human Services, or any related field
Minimum of two years of experience in case management, social services, or child welfare.
Experience working with children with special needs, emotional disorders, or developmental disabilities preferred.
Knowledge of trauma-informed care and Trust-Based Relational Intervention (TBRI) preferred.
Valid driver’s license and reliable transportation.
