JOB TITLE : INFECTION CONTROL / WOUND LICENSED PRACTICAL NURSE
DEPARTMENT : NURSING SERVICE

ESSENTIAL FUNCTIONS :
- Presents self as a role model to achieve the mission of Rest Haven-York. Abides by Rest Haven-York and state / federal regulatory policies and procedures.
- Attends rounds each morning and tracks all residents with signs and symptoms of potential infections and residents diagnosed with an active infection.
- Monitors and updates Infection Control Line List and Precautions List : collects data and maintains records in electronic health record for residents with infections.
- Tracks residents with catheters and suprapubic tubes and completes required documentation and care planning in electronic health record.
- Updates care plans and care profiles for new infections and resolved infections.
- Communication with Medical Records to ensure implementation and resolution of diagnosis codes.
- Ensures placement of orders for nursing staff to monitor active infections.
- Follows and participates in development of policies to prevent infections.
- Implements and maintains isolation procedures for residents with contagious infectious diseases and on enhanced precautions; including ensuring supplies and signage are in place.
- Ensures completion of transfer communication form for all residents with a MDRO.
- Ensures all new admissions have completed immunization consents. Obtains immunization history and enters into electronic health record.
- Tracks immunizations for residents and staff as required by CMS and the PA DOH. Responsible for maintaining unit and staff records with up-to-date information and reporting information as required per regulations.
- Responsible for reporting and notifications required by the PA Patient Safety Reporting Authority.
- Organizes and delegates duties for FLU season each year, including ordering and receiving FLU vaccine for residents and staff.
- Works in collaboration with pharmacy to conduct immunization clinics.
- Manage and track TSTs as needed.
- Oversees the prevention, assessment and care of wounds in the facility.
- Works in collaboration with wound CRNP and attends weekly wound rounds. Responsible for obtaining new orders as recommended by wound CRNP.
- Updates care plans and care profiles for residents with wounds, catheters and ostomies.
- Follows and participates in development of policies and procedures for wound care,
- Oversees the care of residents with ostomies in facility, ensuring needed supplies are available.
- Responsible for monitoring treatment sheets in treatment books to ensure completion of weekly body checks and correct documentation of alterations in skin integrity. Ensures completion skin wound documentation forms for new alterations in skin integrity.
- Collaborates with MDS Coordinator / RNAC and provide needed documentation for completion of residents MDS
- Order and ensure availability of wound and treatment supplies in central supply and on nursing units, working in collaboration with inventory specialist to monitor cost.
- Tracks use and location of APM2 mattresses.
- Assists Staff Development Coordinator in conducting new staff orientation as needed and scheduling new nursing staff to attend wound rounds during orientation.
- Educates staff as needed.
- Maintains up to date information about changes in infection control and wound care practices. Attends seminars and educational opportunities as needed.
- Attends Quarterly QA Meeting and reports on all trends, patterns, and potential problem areas for previous quarter. Completes QA / QI audits for infection control and wound care.

- Attend trainings to build understanding and capacity to undertake QAPI work as needed.
- Carry out QAPI roles and responsibilities as assigned.
- Follow established policies and procedures in support of QAPI efforts.
- Look for and share with leadership ideas for improvement in the organization.
- Communicate to leadership upon witnessing a positive outcome of a QAPI project or detecting barriers preventing project success.
- Support QAPI efforts both verbally and non-verbally (i.e., via actions and attitude). Including performance and practice in accordance with QAPI initiatives and findings.
- Recognize your role for Trauma Informed Care : Promotes a culture of safety, empowerment and healing; recognizes that everyone who interacts at Rest Haven may have past traumatic experiences and protects trauma survivors from being re-traumatized.
- Recognize your role for Compliance and Ethics : Participates in annual compliance and ethics training. Follows code of conduct reporting any compliance and ethics concerns.
