Job Overview :
The Housing Case Manager provides clinically appropriate and comprehensive case management services to individuals in the court system. This position is responsible for working directly with participants to assess, plan, implement, coordinate, monitor, and evaluate the options and services needed to assist with meeting the participants' housing-related needs.
Responsibilities / Duties :
Build relationships with participants, staff, and volunteers by treating all individuals with respect, safety, and dignity.
Build community relationships with the Wellness Court and Re-entry Court personnel by attending all required staff meetings with the Courts.
Communicate with the treatment team, probation and parole officers, and court representatives in order to plan, coordinate services, and relay information on clients' progress.
Assist participants in the development of participant-driven housing plans with identified short- and long-term goals.
Empower participants to become involved in their own planning and goal setting.
Balance the needs of the caseload based upon acuity, length of time in program, state of housing stability, etc., supporting 10-25 households at a time.
Assure that specific HUD program goals are consistently being met and provide appropriate levels of case management support to foster transition to the next stage of housing.
Input timely data into HMIS database to meet funding requirements and statistical reporting. Complete daily notes, case management logs, and all other specific requirements in a timely manner.
Approximately twenty-five (25) hours of direct or indirect participant service contact weekly is required.
Other duties as assigned.
Bending, stooping, walking, standing, sitting, and reaching are required. The team member must be able to lift 25 lbs.
Be able to travel between locations for meetings and training throughout the local community.
Qualifications :
A Bachelor's Degree in Social Services or Human Services related field preferred.
A minimum of three (3) years of experience working with individuals, families, and / or youth who may experience severe and persistent mental illness, substance abuse, trauma, domestic violence, and / or extreme poverty. Five (5) years of experience is preferred.
A minimum of 2 years of experience working with Specialty Courts (Drug Court, Mental Health Court, Probation, and Parole).
Knowledge of and the ability to interpret and apply laws, rules, policies, and procedures relating to housing assistance programs.
Adapts to changing business needs, conditions, and work responsibilities; responds to change with a positive attitude and a willingness to learn new ways to accomplish work activities and objectives.
Demonstrates responsible personal and professional conduct; accepts personal responsibility for the quality and timeliness of work.
Applies effective written and verbal communication techniques to convey clear and timely expectations, listens to people's ideas and concerns, resolves confrontations, disagreements, and complaints in a constructive manner.
Ability to exercise independent judgment, using analysis, experience, and perspective in solving problems or communicating decisions or actions.
Effective interpersonal skills, with the ability to work collaboratively to garner trust, respect, and confidence from others.
Ability to work effectively under pressure in a fast-paced environment.
A collaborative team member who is also a strong leader with well-developed organizational and time management skills.
Ability to develop effective working relationships; works cooperatively, exchanges ideas, and addresses issues in a constructive manner.
MS Office experience, especially Outlook.
Must pass a criminal records check and pre-employment drug screening.
Must have a valid driver's license and current automobile insurance.
