Essential Responsibilities
- Utilize the resident service plan and resident story to provide personalized and appropriate care for each resident.
- Assist with personal care, ambulation, transfers, and positioning as needed while maintaining the privacy, comfort and dignity of each resident.
- Respond promptly to the notification system signaled by the resident.
- Administer medications as directed by the physician.
- Monitor, document and report changes in the resident’s condition to the clinical support team.
- Monitor, intervene and report unsafe conditions and document incidents and or accidents when they occur.
- Report any actual or suspected waste, fraud or abuse to the Director and/or designee immediately.
- Perform light housekeeping duties and assist with serving meals, snacks and beverages.
- Communicate openly and honestly at all levels and work as a cohesive team to create and maintain an environment in which everyone can thrive.
- Engages residents in unplanned and planned activities supporting resident’s psychosocial well-being.
- Provide excellent customer service, support and encouragement to residents, families, coworkers and other customers through the Resthaven Way culture.
- Attend and participate in orientation programs, on-going training and educational classes.
- Follow established safety precautions, infection control protocols and maintain privacy and confidentiality as required by Resthaven’s Corporate Compliance program.
- Follow established smoking regulations and report violations.
- Completes incident reports and related paperwork a necessary.
- Perform other related duties as assigned and/or requested by the resident.
- Basic computer skills.
- Effective organizational skills.

- Ability to remain calm and make prompt and accurate judgments with regard to resident care and emergencies.
- Ability to collaborate and communicate honestly and effectively as part of the work team.

- Ability to set priorities, adjust to fluctuating schedules/assignments and multi-task.
- Completion of HS Diploma or GED strongly preferred.
- Previous related experience or training preferred, especially working with the elderly with memory loss.
- Medication pass training.
- Dementia training.
- Practice high standards of ethics, honesty, and accuracy.
- Ensure resident/customer satisfaction.
- Flexible schedule with ability to work weekends and holidays.
- The employee is required to stand; walk; sit; handle or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; pull, push, twist, turn, bend, stoop, kneel, or crouch; smell; talk and hear.
- The employee must be able to lift and/or move up to 50 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
