Restoration Medicine is a Virtual Care startup based in the Roux Institute in Portland Maine. We deliver care for patients with a wide range of pelvic health challenges. We are building our clinical team in Maine. This position starts in a part time/training mode, with the opportunity to move to full time as we grow.

About the Role: As a Nurse Navigator, you will play a vital role in guiding patients through their healthcare journey using Restoration Medicine's personalized care plans. You will focus on ensuring that patients receive the care they need, educating patients, coordinating Restoration Medicine’s clinical team with outside resources.
Key Responsibilities:
- Educate patients and families about their conditions, treatments, and role in recovery.
- Communicate with the patient’s multidisciplinary team in execution of the care plan.
- Advocate for patients by ensuring appropriate levels of care and providing resources.
- Involve patients, PCPs, family caregivers and social workers if necessary in the process.
- Enable tele-health monitoring to manage high-risk patients effectively.
- Provide direct patient care and assessments as needed.
- Maintain accurate records of communications, interventions, and management reports.
- Ensure compliance with regulatory standards (DPH, ERISA, etc.)
- Adhere to HIPAA/HITECH guidelines.
- Assist patients with paperwork, and appointment coordination to reduce barriers to care.

Qualifications:
- Required: Maine RN Licensure and completion of an accredited nursing program with a Master’s or Doctoral degree.
- Preferred: Bachelor of Science in Nursing (BSN).
- Required Experience: Pelvic Health (Urogyn, Bowel, Neuro, Pain) and clinical acumen
