Position Overview
As part of the Reentry team, the System Navigator/CalAIM Case Manager provides support and accompaniment to people returning to San Luis Obispo County from incarceration through case management and CalAIM programs. Key responsibilities include, but are not limited to be the subject matter expert of CalAIM Enhanced Care Management and Community Supports. This role requires strong communication skills, empathy, and commitment to Restorative Partners’ mission.
Roles & Responsibilities
Client Support and Case Management
- Maintain an average caseload of 50 ECM program participants
- Conduct initial outreach and engagement to establish rapport, determine program eligibility, ensure participant consent to services, and secure CenCal ECM program enrollment and authorization
- Ensure participants are assigned a Primary Care Physician and have access to consistent care
- Conduct intake and individualized needs assessments to develop plans that addresses participant's needs in the areas of physical health, mental health, SUD, community-based Long-Term Services Support, oral health, social supports, Social Determinants of Health, and housing goals
- Participate in RP’s regular interdepartmental meetings to collaborate and coordinate with other team members or regarding care plans, supports, and referrals for participants
- Support participant’s engagement in coordination of scheduling appointments, appointment reminders, coordinating transportation, accompany participant to critical appointments, identify and address other barriers to participant’s engagement in services
- Coach members to make lifestyle choices based on healthy behavior
- Support members in strengthening their skills to identify and access resources to assist them in managing and prevention of chronic condition
- Identify supports needed for participant
- Assist members in accessing additional benefits and related documentation such as, Social Security Insurance (SSI), CalFresh, cash aid, and obtaining required documentation to apply (ID, birth certificate, proof of medical conditions, etc.)
- Network with community and stakeholders to remain current on issues and activities as they impact coordination of care for participants
- Maintain confidentiality of participant files per HIPAA and all applicable guidelines
Community Engagement & Education
- Participant outreach and engagement, including direct communication with participants such as in person meetings, mail, email, texts and telephone; community and street-level outreach
- Build and maintain relationships with local organizations, healthcare providers, and government agencies to create a strong referral network
- Represent the organization at community events and meetings to raise awareness about our services
- Assist with training volunteers and/or interns to support program activities and client services
Administrative Duties
- Track case notes and referrals in Apricot, RP’s internal database, and conduct follow-ups as required
- Prepare and submit regular reports on program activities, client progress, and outcomes
- Participate in staff meetings and contribute to the development of organizational policies and procedures
- Support tracking and reporting for CalAIM invoicing
- Assist in providing relevant data and success stories
Professional Development
- Stay updated on best practices in social work and relevant fi elds through ongoing education and training
- Attend required workshops, webinars, and conferences to enhance skills and knowledge
- Participate in regular supervision

Essential Requirements
- Valid government-issued identification
- Current driver's license, valid auto insurance, and reliable transportation
- Successful completion of a comprehensive background check, as verified by law enforcement administration or other designated agency
- Compliance with organizational Mandatory Vaccination Policy
- Bachelor's degree in Social Work, Psychology, Criminal Justice, or related fi eld preferred
- Minimum of 2 years experience in restorative justice practices or related field preferred
- Proficiency in Google Suite, Microsoft Office Suite, and database management systems
- Ability to work flexible hours, including occasional evenings and weekends
- Fluency in English (bilingual English/Spanish highly desirable)
